Your patient portal is a secure online website that gives you access to your personal health information and medical records from anywhere, at any time. This can help you take a more active role in your health care.

Ask your office for access to Patient Portal today! After you gain access, you can click the button below to sign into Patient Portal.

How to Register

When you ask for access, our staff will create a username and password for you. Click the link for the patient portal and select Patient Login. Enter the username and password created for you. You will be prompted to change your password the first time you login.

Appointments

In the appointments tab you can view Upcoming appointments, Video Visits, Past appointments and results. Select a visit date to see the finalized visit note, patient education or handout materials.

Accessing Test Results

To access test results, click on the Tests & Results tab. Select the date to view the pathology report. You can select the information bubble to view more information on the diagnosis. If you have questions or concerns about the result, click Compose to contact the provider.

How to Send Messages or Request Refills

With Patient Portal, you can quickly contact your provider to ask questions or request refills. Under the Messages tab, click Compose Message.
